First Visit

Your initial appointment will usually consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation, however, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment.

Please assist us with the following:

  • Your Referral Slip and X-Rays, if applicable. Please have your referring dentist email them, at least one week prior to your appointment, to:  [email protected]
  • Please read our Office Policies and complete your Patient Registration at least one week prior to your appointment.  Please include current medications and detailed insurance information. Your ID#, policy holder name & date of birth, claims address & phone # is required for us to submit your claim.
  • Bring a Photo ID and your Insurance Card (if applicable) to your appointment.  

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

For patients undergoing IV or General Anesthesia, please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.

Please alert the office, prior to surgery, if you have a medical condition that may be of concern (i.e., diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are taking the following medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
